Rehab first after a fracture, not an ALF apartment
Hospital discharge planners almost always start with a skilled nursing facility. Daily physical therapy, incision or nail-site checks, blood-thinner timing, and a new walker belong there. Medicare may cover a qualifying short skilled stay after an inpatient admission. That clock is rehab. It is not a long-term assisted living lease.
When therapy goals are met, or progress stalls, the facility issues a Notice of Medicare Non-Coverage. Build the landing place earlier. If the person can transfer with the help the community will actually provide, follow the written weight-bearing rule, and keep the incision clean, a licensed ALF can be the next address. If they still need 24-hour nursing, a mechanical lift the community will not staff, or daily skilled therapy, stay on the nursing-home hub. Rule 59A-36.006 generally blocks admission when unfinished skilled rehab is required.

The tour is weight-bearing, toilets, and a second-fall plan
Ask whether staff will cue non-weight-bearing or toe-touch rules at 2 a.m., not only during the daytime activity hour. AHCA Form 1823 should name the fracture date, the hardware, the weight-bearing status, and the walker or wheelchair. Extended Congregate Care or Limited Nursing Services can be the difference after a setback. A standard license is not a rehab gym.
