CMS rates Medicare/Medicaid certified nursing homes on a one-to-five star scale for overall quality, health inspections, staffing, and quality measures. Stars are a starting filter, not a substitute for touring and reading survey details.

A five-star overall rating can still hide a recent serious survey issue. A three-star home near family with strong short-stay rehab outcomes may fit better than a distant five-star campus.
