Assisted Living After Open-Heart Surgery in Florida
Open-heart surgery (CABG, valve repair, or valve replacement) sends families to the same search as a hip fracture: assisted living after hospital discharge. The first decision is not which Florida campus looks nicest. It is whether the next address still needs daily skilled therapy and wound checks, or safer nights once the person is medically stable. Rehab first, then a home with help
Hospital discharge planners almost always start with a skilled nursing facility or home health, not an ALF apartment. Walking distance, sternal precautions (no pushing up from a chair with both arms), incision checks, and a new heart-medication list belong in rehab or a home-health episode. Medicare may cover a qualifying short skilled stay after an inpatient admission. That clock is rehab. It is not a long-term assisted living lease.
When therapy goals are met, or progress stalls, the facility issues a Notice of Medicare Non-Coverage. That is the week families rush. Build the landing place earlier. If the person can walk a safe distance, transfer without using the chest for leverage, and swallow the current pills, a licensed ALF can be the next address. If they still need 24-hour nursing, daily skilled therapy, or unstable fluid overload, stay on the nursing-home hub. What a Florida ALF can and cannot keep
Ask who reminds staff not to pull the person up by the arms. Ask how the kitchen handles a low-salt plate. Ask who administers evening beta blockers and anticoagulants if a valve is in place, not only "we cue medications." Oxygen plus a weekend storm is a Florida question. Extended Congregate Care or Limited Nursing Services can be the difference between staying after a flare and transferring. Every Florida ALF also needs AHCA Form 1823. Ask the surgeon or rehab provider to start it before discharge so paperwork is not the delay.
Shortlist while the person is still in therapy
Open the nearest Florida city hub and the nursing home hub. Compare two ALFs that can take the current cardiac picture, plus a skilled option if therapy is still the driver.
