Medicare Home Health in Florida Assisted Living
Families hear "we will send home health" and assume Medicare will also pay the assisted living month. It will not. What Medicare can do is send intermittent skilled visits to the ALF apartment when the person qualifies. This page is not billing or legal advice. The ALF can be home. The benefit is still narrow.
CMS treats an assisted living apartment as a home for the home-health benefit when the person is under a doctor's care, needs intermittent skilled nursing or therapy, is homebound, and uses a Medicare-certified agency. Homebound does not mean never leaving. It means leaving takes considerable and taxing effort, with room for medical appointments and infrequent short outings.
Intermittent means part-time. It is not a night shift. Covered visits can include skilled nursing, physical therapy, and related aide time under a skilled plan. They do not include room, board, or the ALF care-level fee. If you need someone awake all night, that is private duty or a different setting. Read those pages before you decline a nursing-home rehab stay.
Home health is also not a license workaround. If Rule 59A-36.006 says the person cannot stay (24-hour nursing, staff-assisted tube feeding, blood products in the building), an outside nurse a few times a week does not fix that. The administrator still decides continued residency.
Use it on the week Medicare rehab ends
When a skilled-nursing stay is ending, ask the discharge planner whether the next address is an ALF plus home health, or a longer skilled stay. Start AHCA Form 1823 before the Notice of Medicare Non-Coverage. Name the Foley, the wound, or the post-chemo weakness on that form. Then open a Florida city hub and compare two ALFs that already work with Medicare-certified agencies.

- Yes, when you meet the usual Medicare rules: a physician order, intermittent skilled need, homebound status, and a Medicare-certified agency. The assisted living apartment can count as home. Medicare still does not pay the ALF rent.

- No. Home health is intermittent nursing or therapy, not 24-hour coverage. Facility staff still handle daily care the license allows. A few skilled visits a week do not keep someone who needs round-the-clock licensed nursing.
- Is Medicare home health the same as a private-duty aide in the ALF?
- No. Home health is a skilled, time-limited Medicare benefit. Private duty is usually family-paid hours. Some residents use a home-health nurse for the wound or catheter and a private aide for extra evenings. Read both guides before you budget.
