What to ask before you treat the ostomy as a yes
Ask who changes the wafer, how often, and whether that person is on the floor at 2 a.m. Ask whether pouches and barrier rings are in the base rate or billed per item. Ask what happens if the stoma leaks onto a wound or the person cannot tell staff the bag is full. Incontinence plus an ostomy is a skin plan, not two separate add-ons.
AHCA Form 1823 should describe the ostomy type, how long it has been in place, and who maintains it. An independent resident who changes their own appliance is a different admission than total assist after last week's surgery. Extended Congregate Care or Limited Nursing Services can matter if the surgeon still wants skilled checks.
When the license, not the appliance, is the limit
If the discharge summary also lists a feeding tube, suctioning, IV antibiotics, or stage pressure injuries, do not tour standard ALFs first. Start on the nursing home hub or read those condition pages. A dining room that will "work around" a bag is not a clinical plan.

- Not always. Pouches, wafers, and extra changes are often billed as supplies or a higher care tier. Ask for a written all-in quote that includes appliance changes on nights and weekends, not only the advertised starting rent.
- When does an ostomy mean a nursing home instead of assisted living?
